The Dominican Republic is a middle-income country with an overall HIV prevalence of 0.8%. The country has the second-highest number of people living with HIV in the Caribbean. The epidemic is mostly concentrated in key populations, including men who have sex with men, transgender populations, sex workers, and Haitian migrants. NGOs serve nearly 40 percent of people living with HIV on treatment under the national HIV care program and have historically relied on donor funding to provide these services. As donors prepare to withdraw their financial support for the national HIV response, NGOs have to identify new domestic revenue sources from the public and private sectors. <\/p>\n\n

  SHOPS Plus strengthened the efficiency and sustainability of four NGOs by supporting their transition to social enterprise business models and their access to new revenue sources.   SHOPS Plus identified institutional barriers that hindered the private sector in its provision of HIV services, including the lack of a formal mechanism to participate in the national HIV response, and advocated to government partners to address them.   SHOPS Plus identified four NGOs that were at risk due to declining donor funding but well-positioned to transition to social enterprise-oriented models, which use a market-driven approach while focusing on social good.   To secure a sustainable source of domestic revenue, SHOPS Plus supported five NGOs contract with SENASA to access insurance payments for HIV services. Contracting with SENASA requires that NGOs comply with strict requirements for infrastructure, equipment, human resources, and certifications. SHOPS Plus guided NGOs through the process and advocated with SENASA to contract these NGOs. SENASA approved five NGOs for contracting and successfully signed contracts with three \u2013 a first for NGOs in the DR. The contracts ensure that NGOs can receive payment for HIV services without having to charge people living with HIV. <\/p>\n\n

  Contracting with SENASA provides NGOs with the opportunity to continue offering HIV services at no cost; however, people living with HIV must first register with the national health insurance scheme before the insurance company will pay the NGOs for their care. Stigma and discrimination, documentation requirements, accessibility and other barriers prevent people living with HIV from enrolling in social health insurance. Through a series of discussions with four key government partners, SHOPS Plus developed standard operating procedures and had partners sign an agreement to institutionalize the process for enrolling people living with HIV in the health insurance scheme. The SOPs outline every process from application through client notification of enrollment.<\/p>\n\n

  In the DR, there is currently no formal mechanism for private providers to apply to participate in the national HIV care program. There is also no system in place for the government to monitor the quality of private providers already in the program and assess the suitability of new providers wishing to participate. SHOPS Plus is supporting the government to develop transparent, formal, and competitive processes for accrediting HIV services offered by private providers. These processes will ultimately increase the number and improve the quality of NGO and private for-profit providers formally engaged in the national HIV response.<\/p>\n\n

  Including antiretroviral drugs in the Family Health Insurance benefits package would ensure secure, sustainable domestic financing for these lifesaving commodities. This process would shift financing of antiretroviral treatment from the Ministry of Health to the Social Security Treasury, which would free up the Ministry's resources to finance other HIV response priorities like prevention. SHOPS Plus supported the submission of a proposed resolution to integrate ARVs into the Family Health Insurance benefits package and developed a plan to implement the resolution. The project is now working with stakeholders to adopt materials as needed to begin implementation. <\/p>\n\n

  SHOPS Plus supported the development of efficient and sustainable markets for HIV self-testing kits. Globally, these kits help expand access to HIV testing services, especially for key populations who have low uptake of other testing options. The project completed a landscape assessment and worked with in-country partners to identify market gaps and opportunities and presented findings during a stakeholder workshop. <\/p>\n\n
